Required Policies and Procedures
Overview
Organizations enrolling as a CHW Agency MUST submit required policies and procedures as part of the enrollment process.
Steps for Organizing and Submitting CHW Policies and Procedures
Step 1: Review Required Policies and Procedures
The following policies are required to be submitted to SD Medicaid as part of the provider enrollment process.
- Abuse and Neglect Reporting Policy
- Staffing Policy
- Staff Training Policy
- Intake/Admission Policy
- Discharge Policy
- Eligible Recipient Confidentiality Policy
- Recipient Rights and Responsibilities Policy
- Documentation Policy and Procedures
- Incident Reporting Policy
- Emergency Response Policy
- Health and Safety Including Universal Precautions Policy
- Quality Assurance Policy
- Eligible Recipient Grievances Policy
- Gifting Policy

Step 2: Gather and Review Current Policies and Procedures
Many organizations likely have most, if not all, of the required policies and procedures already in place. Organizations should work to gather the policies and procedures that they have and then review to ensure they meet the requirements listed in the CHW Agency Addendum.
If an organization does not have a required policy or procedure, the CHWSD has worked to draft sample policies and procedures that can be reviewed and approved by the organization.
Step 3: Internal Policies and Procedures Approvals
Organizations that do not have all required policies and procedures should work internally within their organization to get approval to adopt the new and/or updated policies. This may require board approval, policy committee approval, or other internal approvals. This process is unique to each organization and organizations are responsible for getting all new and/or updated approvals adopted internally.
Step 4: Submit Policies and Procedures to SD Medicaid
Policies and procedures should be submitted to SD Medicaid for review in the same order as the CHW Agency Addendum and submitted as one PDF document with clearly labeled headers for each policy. Policies and procedures should be emailed to SDMedXGeneral@State.SD.US.
