Technical Assistance and Support

Overview

Through funding from the South Dakota Department of Health, as part of the Rural Health Transformation Program, the CHWSD is able to provide Technical Assistance and Support to any organization that currently provides direct patient/client services and is looking to start, expand, and/or sustain a CHW or CHR program.

As the funding source for CHWSD Technical Assistance and Support has changed, all CHW/CHR programs receiving support need to complete this short form in order to continue to receive support. This allows the CHWSD to track grant metrics and continue to track sites progress as they work to grow, support, and sustain a CHW or CHR program.
