Overview

To assist in developing and submitting a comprehensive budget, the following sample budgets have been provided to guide your site and organization in completing the project budget.

1 CHW

1 CHW and CHW-Led Evidence-Based Programming

This sample budget provides guidance for developing a budget to develop one (1) full-time CHW position.
This sample budget provides guidance for developing a budget to not only support one (1) full-time CHW position but also providing financial support for the implementation of a CHW led (or co-led) evidence-based program, such as Better Choices, Better Health® SD (BCBH-SD) and/or the National Diabetes Prevention Program (National DPP). 1 Community-Based CHW

1 Cross-Trained, Community-Based CHW

This sample budget provides information about how a Community-Based Organization (i.e. a social services or non-profit organization that may not choose to bill a payer for CHW services) can stretch the budget to support the development of a sustainable CHW program.
For smaller Community-Based Organizations that may not be able to support or sustain a full CHW position, there is an opportunity to cross-train a staff member to also work as a CHW. This sample budget provides information for a cross-trained budget.
