Booth Information:

Please complete the following form to be considered for an educational booth at the upcoming April 23 and 24, 2025 SD CHW Conference. Due to limited space, approximately 20 booths will be available. Please only consider submitting a proposal for an educational booth if your organization provides education to CHWs/CHRs and/or provides regional or statewide resources. Notice of Selection

Notice of selection will be provided by April 1, 2025, to allow organizations adequate planning for the event.

• The fee for non-profit organizations is $250 and includes 2 attendee registrations.
• The fee for for-profit organizations is $400 and includes 2 attendee registrations.

Sponsor logos and program ads are due to CHWSD by March 1, 2025, at the latest to ensure printing prior to the conference. Each organizations will be asked to provide a door prize with a value of approximately $50. Attendees will receive a passport to have booths sign off on to be eligible to participate in the door prize drawing.
