Booth Information:

Please complete the following form to be considered for an educational booth at the upcoming April 29 and 30, 2026, SD CHW Conference and/or the upcoming October 14, 2026, Annual Fall Forum. Please only consider submitting a proposal for an educational booth if your organization provides education to CHWs/CHRs and/or provides regional or statewide resources. For questions or concerns regarding the booth proposals, please email info@chwsd.org.

Notice of Selection

Notice of selection will be provided by March 1, 2026, for the April conference and September 1, 2026, for the October conference to allow organizations adequate planning for the event.

An option will be given to have a booth at one conference (select which one) or for both conferences.

• The fee for non-profit organizations is $350 for one conference or $700 for both conferences (includes 2 attendee registrations for each conference).
• The fee for for-profit organizations is $500 for one conference or $1,000 for both conferences (includes 2 attendee registrations for each conference).

Sponsor logos and program ads are due to CHWSD by March 15, 2026, at the latest for the 5th Annual Conference, and by September 15th for the 12th Annual Fall Forum to ensure printing prior to the conference. Each organizations will be asked to provide a door prize with a value of approximately $50. Attendees will receive a passport to have booths sign off on to be eligible to participate in the door prize drawing.
