Proposal for Educational Booth
Booth Information:
Please complete the following form to be considered for an educational booth at the upcoming May 15 and 16, 2023 SD CHW Conference. Due to limited space, approximately 30 booths will be available. Please only consider submitting a proposal for an educational booth if your organization provides education to CHWs/CHRs and/or provides regional or statewide resources. Notice of Selection
Notice of selection will be provided by April 1, 2023, to allow organizations adequate planning for the event. There is no fee for a conference booth, but organizations will be asked to provide a door prize. Attendees will receive a passport to have booths sign off on to be eligible to participate in the door prize drawing.
