What is the PRAPARE Screening?Â
Protocol for Responding to & Assessing Patients’ Assets, Risks & Experience or PRAPARE is a national standardized patient risk assessment protocol designed to engage patients in assessing & addressing social determinants of health (SDoH). PRAPARE is evidence-based, designed through stakeholder engagement, paired with an Implementation and Action Toolkit, and standardized across ICD-10, LOINC, and SNOMED.

Using a SDoH Screening Tool
PRAPARE is just one of many SDoH screening tools available to address social determinants of health with patients/clients. Numerous other tools and customized versions of the PRAPARE screening are often readily accessible for utilization within an organization’s Electronic Health Record (EHR) system.
The process of screening for SDoH can take place either through traditional paper-based methods or electronically, where Community Health Workers (CHWs) can pose the questions to the patient or client directly, or the patients can complete the screening themselves, either electronically or by filling out a physical copy of the screening questionnaire.
