Application Checklist
Although the application process is designed to be simple, there are still steps that need to be followed to ensure your application is submitted correctly. Follow the steps below to ensure your application is reviewed in a timely manner.
Access and complete the CEU version of the South Dakota CHW Planning and Assessment Toolkit.
The CEU version allows you to receive CEUs (if needed) and to complete the toolkit and receive a general certificate of completion. You will need to complete the toolkit and upload your certificate of completion or CEU certificate as part of the application.
Download the budget template and review the project narrative questions.

Work with others within your organization to draft project narrative.
Although this is a simple application, a project narrative is still required as part of the application. Respond to the project narrative prompts and questions using the fillable form.

Work with others within your organization to create a project budget.
Use the budget template to create a project budget. To assist sites and organizations in developing a project budget, sample budgets are available for review here.

Complete the online application and upload budget.
Once you have reviewed the project narrative questions and drafted your program budget, you are now able to submit your application. The application is a simple form that requires basic information about your site and organization (i.e., organization name, address, contact email, etc.) and also requires you to respond to project narrative questions, upload your CHW Toolkit Certificate of Completion or CEU Certificate, Project Budget, and Project Narrative.
