Overview
Community Health Workers (CHWs) are able to provide Targeted Case Management services for Justice-Involved Youth. The following resources can be used by individuals and organizations providing Targeted Case Management services. These templates were developed by the CHWSD through a contract with the South Dakota Department of Social Services, Division of Medical Services (SD DSS DMS). They are intended as general examples and are subject to change. Templates will be reviewed and updated as requirements, policies, or guidance change.
